For getting your child younger than age 7 baptized at Holy Name, follow the five steps below. For those older than age 7, click here.
Step Two - Godparents (Sponsors)
You may have one or two Godparents. If you have two Godparents, they must be one male and one female. At least one of your sponsors must be a Godparent that is a registered and practicing Catholic. The second sponsor, if you have one, can be a Godparent or Christian witness. A Christian witness is a non-Catholic Christian sponsor. Godparents are welcome to attend our baptism class or a class at their home parish. We do not require the class for Godparents. All Godparents must complete a Godparent Eligibility Form, signed by their pastor and return it to us within two weeks of the scheduled baptism date.
Step Three - Take the Baptism Class
Parents need to attend a baptism preparation class. Godparents are encouraged to attend as well. In preparation for the baptism class, we request that couples watch the three Reborn video segments on the Formed website. The website may be found at formed.org. (Register on the site and enter our parish code K6THYD to access its content. We provide a one hour class for parents and Godparents to prepare for baptism, the first Sacrament of Initiation into the Catholic Church. Step Four - Schedule the Baptism
We normally offer baptisms during our regular weekend Masses:
- Saturday Spanish/English Mass at Holy Name Church at 5pm
- Sunday Mass at Holy Name Church at 8:30 AM
- Sunday Mass at St Martin’s Church in Oak Creek at 11:00 AM
- Sunday Mass at Holy Name Church at 5:00 PM
